Responsible for understanding the Clinical programs, identifying practice opportunities and integrating clinical programs and resources into the practice workflows with the intent to optimize the patient experience, quality of care provided, and positively impact resource utilization, including decreasing readmissions and unnecessary ED visits. The Manager will support HHC inpatient services and physician practices by collecting data and creating reports as directed to analyze performance of PH efforts and inform decision-making, process and program implementation successes in our primary goals to improve coordination of care amongst providers and improve Chronic Disease Management for our patients. Metrics of success will include but are not limited to decreased patient readmissions after hospital stays, decreased unnecessary ED visits, improved metrics of Chronic Disease Management per CMS standards as delineated in CMS shared risk, shared savings programs with decreased costs of care, thus improving care Value. Responsible for the daily operations of the PH staff and providing oversight and assistance with all aspects of the PH processes, including aiding in collaboration with HHC Case Management, Social Work and Discharge Nursing with Home Health agencies and physician practices to improve member/patient outcomes and costs by more effectively managing Transitions of Care. Responsible for preparation of reports for the Director of Population Health, HHC CMO, HHC CEO, TC2 and the HHC Population Health Committee. Develop plans to meet objectives. Identify and resolve operational problems using defined processes, expertise and judgment. Decisions are guided by policies and procedures. Meeting deadlines is a must. Coordinate, supervise and is accountable for the daily activities of the team. # Undergraduate or higher degree in a health-related field to include education and training relevant to achieve required## experience listed below. # Three (3) years# experience in the healthcare industry. Three (3) years# experience with healthcare Quality Management, Case Management or Utilization Management and regulatory standards. Previous Supervisor experience. Demonstrated ability to organize and manage multiple ongoing projects within expected timeframes. Must show attention to detail and take responsibility for completion of assigned projects within deadlines. Professional verbal and written communication skills, including the ability to develop correspondence to facilities and clinicians. Intermediate level of proficiency with MS Excel and Word.
