Group is seeking a compassionate and skilled Diabetes Educator
(Outpatient) to enhance our commitment to exceptional patient care.
In this role, you will empower individuals with diabetes to
effectively manage their condition through tailored education,
ongoing support, and evidence-based care plans. Working closely
with our healthcare providers, you will play a vital role in
improving health outcomes and quality of life for patients across
our southeast communities. Requirements: Key Responsibilities:
Deliver individualized and group education sessions on diabetes
self-management, covering topics such as nutrition, medication
adherence, blood glucose monitoring, and lifestyle modifications.
Collaborate with physicians, dietitians, and other healthcare
professionals to create and implement personalized care plans that
address each patient’s unique needs. Assess patient needs,
establish measurable goals, and monitor progress through consistent
follow-up appointments. Stay informed about the latest advancements
in diabetes care and treatment, sharing knowledge with patients,
colleagues, and staff as appropriate. Ensure accurate documentation
of patient interactions and care plans, adhering to HIPAA
regulations and organizational policies. Engage in community
outreach initiatives to promote diabetes awareness, prevention, and
education within the Middle Georgia region. Qualifications: Active
license as a healthcare professional (e.g., RN, RD, PharmD, or MD).
Certified Diabetes Care and Education Specialist (CDCES)
certification preferred, or eligibility to obtain within one year.
Minimum 2 years of professional practice experience in a healthcare
field. At least 1,000 hours of diabetes education experience (or
working toward this requirement). Strong communication and teaching
skills with a patient-centered approach. Knowledge of diabetes
management tools and technology (e.g., CGMs, insulin pumps). Key
physical and mental requirements: Ability to lift up to 50 pounds
Ability to push or pull heavy objects using up to 50 pounds of
force Ability to sit for extended periods of time Ability to stand
for extended periods of time Ability to use fine motor skills to
operate office equipment and/or machinery Ability to receive and
comprehend instructions verbally and/or in writing Ability to use
logical reasoning for simple and complex problem solving Ability to
travel to multiple locations to support business needs as required
